Advanced Disease Management

Advanced Disease Management (ADM) is a patient care that is focused on improving the quality of life for individuals with serious, progressive illnesses. Generally given alongside palliative care. It involves a coordinated effort to manage symptoms, provide education, and support patients and their families through complex medical decisions. 

ADM programs are typically led by a nurse practitioner and involve a multidisciplinary team, including physicians, pharmacists, social workers, and spiritual care counsellors. 

Key aspects of Advanced Disease Management include:

  • Symptom Management:

Understanding the pain and other evident symptoms associated with the advanced illness. 

  • Education and Support:

Providing information about the progression of the illness, available treatment options, and setting expectations, along with support for ensuring informed decisions. 

  • Palliative Care Integration:

Recognizing the importance of palliative care principles in managing advanced disease, thus improve quality of life for both the patient and the family. 

  • Multidisciplinary Team Approach:

Involving various healthcare professionals to address the diverse needs of patients associated with advanced illnesses, where cross consultation maybe required.

  • Focus on Patient and Family:

Prioritizing the patient’s and their family’s needs and preferences throughout the disease management process. 

  • Advanced Illness Care Planning:

Facilitating conversations about goals of care, advance directives, and end-of-life planning.
